APAC

APAC NSCCH is an acute substitution and post acute care service working as a multi-disciplinary team. The services aim is to provide evidenced base care in the patient’s home, hostel, nursing home, clinic or work environment. APAC care is provided by credentialed staff with fully accredited APAC NSCCH area approved Clinical Guidelines and Operational Policies. Clients are admitted to APAC and then provided with the appropriate treatment and care until resolution of their acute phase or by transitioning them onto various other health and support agencies in the community for ongoing care. The post acute component of APAC provides Acute or Chronic patients with treatment, education and support to live with their illness thus reducing hospital unplanned admissions and number of admission per year.

Drug and Alcohol

NSCCH Area Drug and Alcohol provide a range of treatment options for individuals and families affected by alcohol, drugs and gambling. Services include:

  • Intake, advice and assessment
  • Counseling services
  • Inpatient and outpatient detoxification programs
  • Consultation and liaison services to all NSCCH hospitals.
  • Opioid Treatment programs
  • Cannabis treatment clinic
  • Residential rehabilitation program and housing program
  • Magistrates Early Referral Into Treatment Program
  • Community health promotion, outreach education and prevention
  • Specialist D&A medical services
  • Support and education to other health professionals such as GPs
